School Nurse Memo

May 17, 2019

 Dear Parent or Guardian,

The last day of school is May 31, which is fast approaching. Please be sure to pick up your child’s medication from the clinic before the end of the school day on May 31.  This includes prescription and non-prescription medication as well as nebulizer and any equipment you may have brought in for medical procedures.  An adult must pick up these items, as we cannot send them home with the students.

If the medication is not picked up before the end of the school day on May 31, the medication will be disposed of according to Volusia County School Board administration of medication guidelines at the close of that day.

Please use this web site.

(Parent/ Student Health Services) to access the new medication forms and or procedure forms for next school year, which begins on August 12, 2019.  Medication forms, procedure forms, and emergency care plans must be renewed by the parent, at or before the beginning of each school year in order for your child to receive medication or procedures during school hours.  Students carrying their own epi-pen, inhaler or diabetic supplies, are also required to have a new medication form on file each school year.

Make sure you wait until on or after July 1st, 2019 to get authorizations for 2019/2020 school year – anytime sooner will not be accepted for next school year. Please ask your pharmacist to use the manufacturer expiration date so the medication does not expire before the end of the school year.

If you have any questions please contact your child’s school clinic staff.

Have a wonderful, safe, and healthy summer.  I look forward to seeing you next year!

Sincerely, Patricia A. Scott HST
